Referral Information for Healthcare, Social Services, and Criminal Justice Professionals

Programs Available

  1. High Intensity Outpatient Program (HIOP, formerly PHP): Structured day treatment for individuals needing our most intensive outpatient care while living at home, maintaining recovery, and balancing daily responsibilities like work, school, or family. Includes therapy sessions 3–5 days a week for 20 hours.

  2. Intensive Outpatient Program (IOP): A flexible, moderately intensive program focused on building recovery skills through group and individual therapy several times a week. Ideal for those who need ongoing support but require fewer hours of care than HIOP.

  3. Recovery Housing: Safe and supportive housing for individuals transitioning from treatment to independent living.

  4. Cynthia Day Family Center: Specialized residential treatment for women, including pregnant and parenting women and their children.

Patient Eligibility

  • Patients must be motivated to participate in recovery services.

  • Eligibility requirements vary by program and are tailored to meet individual needs (e.g., HIOP and IOP typically serve individuals with substance use concerns; Recovery Housing supports those currently in treatment or preparing for a transition to independent living).

  • Patients do not need insurance to be considered; Harbor Care offers a sliding-fee scale based on income and accepts Medicaid, Medicare, and most private insurance plans.

How to Refer

  1. Contact Harbor Care Intake Team:

    • Call 603-821-7788 or email hope@harborcarenh.org.

    • Provide basic patient information, including substance use history, medical needs, and the reason for referral.

  2. Submit Supporting Documentation:

    • Include any recent medical records, assessments, or treatment recommendations, including details that might impact treatment, such as housing stability and legal issues.

    • Provide any details on co-occurring conditions, medication management needs, or other relevant clinical information, including current mental health status.

  3. Coordinate Follow-Up:

    • Our intake team will connect with the patient to complete the enrollment process and coordinate an initial evaluation.

Harbor Care is distinguished in Patient-Centered Medical Home protocols, the HRSA National Health Service Corps, and HRSA’s Health Center Program. Harbor Care Health & Wellness Center is a Federally Qualified Health Center, a community-based health center that provides primary care and support services to underserved populations.
