At Harbor Care, our Community Health Services team is dedicated to enhancing the health and well-being of individuals and the community through accessible healthcare and social services, tackling barriers to care.

Trusted Community Health Workers

Our Community Health Workers are integral community members who:

Community Outreach

Our Community Health Workers are out in the community, participating in local events, partnering with organizations, and providing easy access to healthcare solutions. They help people discover and utilize services they might not be aware of.

Resource Connection

Health Education

We educate on chronic disease management, prevention, nutrition, and health maintenance. Our team empowers individuals with the knowledge to set health goals, understand health information, and advocate for themselves and their community.

Health Insurance Assistance

Our Certified Application Counselors (CACs) help navigate health insurance options via Medicaid, CHIP, and the Federal Health Marketplace.

  • Enrollment assistance
  • Follow-up for finalizing healthcare plans
