Harbor Care provides high-quality healthcare for all ages, including primary, preventive, dental, behavioral health, and substance use treatment, addressing social determinants of health through integrated services.
Accessible to All
Services are available in most languages through interpreters. Care is offered via telehealth or at two Nashua locations, with a sliding fee scale to ensure access for everyone—regardless of insurance or income. We accept Medicaid, Medicare, and most private insurance for most programs.
Person-Centered Approach
A dedicated care team coordinates lifelong healthcare while connecting patients to additional resources like housing, food, and transportation for holistic well-being.

Dr. Klebenov serves as Medical Director at Harbor Care Health & Wellness Center, bringing decades of experience in family medicine, correctional health, and care for people facing complex medical and social challenges. His approach emphasizes access, dignity, and meeting patients where they are—especially those who have historically been left out of traditional healthcare systems.
