Not everyone can live alone, and certainly not everyone can afford 24/7 supportive care. Harbor Homes and Harbor Care have several housing programs to keep people in their communities, healthy and connected, with the 24/7 support of staff.

What Is a Housing Program for the Disabled?

Programs focusing on housing for disabled adults in NH are designed to help individuals with disabilities access safe, affordable, and accessible living spaces. These programs often provide financial assistance, modifications, and supportive services to empower participants and enhance their quality of life. Our Housing Programs for Disabled Adults

We have several programs in Nashua for people who cannot live alone due to significant behavioral health disorders or other disabilities. These programs may be a permanent home or a stepping stone on the way to more independent community living. Our programs include:

Group Homes

In our group homes in New Hampshire, individuals find a welcoming residential setting. Each resident has a private bedroom and shared common spaces such as kitchens, living areas, and bathrooms. These group homes for adults with behavioral problems and developmental disabilities are fully licensed and staffed 24/7 to offer personal assistance and a community-oriented lifestyle. This setup encourages sharing and interaction while providing the opportunity for personal space and privacy.

Glencliff to Community Program

A transition haven for elderly individuals, the Glencliff to Community program serves those with significant physical and mental illness who previously resided in state-run facilities like Glencliff. This group setting fosters community and support and allows residents to thrive in a more independent and engaged community environment while receiving the care they need.

Safe Haven

Safe Haven extends a lifeline to individuals with significant disabilities who find themselves without a home and disconnected from community services. Hosting up to five residents, this program focuses on stability—providing a shared space to manage health symptoms and work closely with staff toward the goal of permanent housing. This supportive housing program prepares residents to transition to more independent living arrangements.

NH Hospital Transitional Housing Program

Individuals with considerable behavioral health disorders transitioning from NH Hospital will find solace and support in our NH Hospital Transitional Housing Program. This program is aimed at reintegrating individuals back into the community. It provides the necessary support and care planning, helping residents regain their independence and integrate seamlessly with community activities and supports.

Qualifications, Cost, & Enrollment

These programs have their own eligibility criteria. They do not typically have enrollment applications for the general public; instead, Harbor Homes and Harbor Care staff work with community organizations to fill any vacancies.  Functional Supportive Services

Harbor Care helps clients progress down the road to maximum health and independence. “Functional Support Services” help clients with complex issues—such as significant behavioral health challenges—navigate appointments, maintain themselves and their homes, and stay healthy and happy. How It Works

Our Functional Support Service Providers work directly with clients to help them:

  • Live healthy, through appointment reminders and referrals (to groups, peer support resources and other recovery based activities). 

  • Stay connected with the community, through skills coaching (intrapersonal skills, independent use of public transportation, and independently accessing community resources and services).

  • Live independently, through basic living skills assistance and coaching (e.g., budgeting, cooking, personal hygiene maintenance, self-care, and housekeeping). 

Qualifications, Cost & Insurance

If Functional Services are part of a person’s treatment plan, many insurances will pay for this treatment. Additionally, we offer community members the option to privately enroll in services. For more information, contact us today.

Permanent Supportive Housing

Too many people never really escape homelessness. When struggling with severe and persistent mental illness or substance use disorder, they often move from crisis to crisis; losing a job, getting evicted, losing healthcare is often followed by receiving short-term help. Once this help ends, however, the cycle of homelessness often repeats. Using a “Housing First” model of care, combined with long-term housing and supports, we stop the cycle of chronic homelessness and get people on track to a stable life.  

If you or your family are homeless or at-risk of becoming homeless, please call our region’s coordinated entry line at 1-844-800-9911. This system is not run by the Harbor Care, but will find you a best-fit program, which often is one of ours.

How It Works

Harbor Care often prioritizes its housing for veterans and individuals and families living with disabilities, including behavioral health disorders such as mental illness or substance abuse disorders, physical disabilities, or those living with chronic illness such as HIV/ AIDS. 

Our housing comes in a variety of forms – from shared houses to apartment complexes to “vouchers” that allow a client to choose their own home within the community. In all cases, rent remains affordable, with clients paying a portion of their income towards rent. Our housing often comes bundled together with services such as case management, to help clients find employment, obtain entitlements and benefits for them and their families, and avoid returning to homelessness. 

Programs include:

  • Permanent Supportive Housing Program, for chronically homeless individuals through federal subsidies (non-voucher).

  • Mainstream Voucher Program, for chronically homeless individuals through a federal voucher.

  • Boulder Point, a project-based HUD-VASH (Veterans Affairs Supportive Housing) voucher program for veterans.

Harbor Homes and Harbor Care also operate other federally-subsidized properties, which helps us offer lower rents to qualified community members. Available units are featured on our current housing openings page.

Qualifications, Cost & Enrollment

Each of our housing programs have their own eligibility criteria. Typically clients pay approximately 30% of their income towards rent.
