HIV/AIDS Case Management
HIV is hard enough for anyone to manage. Our HIV/AIDS team makes sure nobody walks that journey alone.

How It Works
For the past 25 years, our HIV/AIDS team has worked directly with individuals living with HIV/AIDS, linking them to the health care and supports they need to maintain good health. In particular, case managers help clients create a plan to:
Access public benefits.
Secure medical care.
Secure stable housing.
Access food.
Access transportation.
Access emergency assistance for rent or utilities, if needed.
For people living with HIV/AIDS, housing is healthcare. Through grants from the US Department of Housing and Urban Development (HUD), in partnership with Harbor Care and the City of Nashua, we are able to ensure individuals who are homeless and living with HIV/AIDS in our community have access to safe, affordable housing.
Locations
If you live in the New Hampshire and are living with HIV/AIDS, we can help connect you with services. Harbor Care case managers work in Greater Nashua (Hillsborough County, with the exception of Manchester) as well as Greater Keene.
Qualifications & Cost
Case management is always at no cost. We serve anybody with an income at or below 500% of the federal poverty level.
