Connect with Our Community Health Services
Our Community Health Services comprise frontline staff who focus on improving individual and community health while providing access to healthcare and social services that support well-being.
Harbor Care’s Community Health Workers are trusted members of the community who connect clients to vital services, partner with social service providers, provide health education information, and create supports designed to help individuals navigate New Hampshire’s social services system while addressing barriers to services.
Outreach
Our Community Health Workers largely operate in the field, offering a low-barrier solution to many healthcare needs.
